Insurance Clients

If you are a Medicare, Pomco, or United Behavioral Health client and are choosing to utilize insurance to cover your sessions, in order to process your claim, you will need to fill out a simple form and provide us with some information. You can save some time by filling out the form for our billing service and submitting it on line right now. Click Here to fill out the form now. If you prefer not to fill this out on line, you will have to fill it out by hand at your first session. You can also call the office and we will mail you a hard form, which you can fill out on your own and send back to us. It will, however, take more time and you will have to wait longer for an appointment.

In order to qualify for insurance, you will also be asked to provide us with your driver’s license and insurance card when you come in for your first appointment, so make sure you bring them with you. Additionally, all insurance co-pays are required by law and must be paid at the session. If you have any questions about whether or not I am a provider on your plan, please call the number for member services on the back of your card. They should be able to give you an accurate, up-to-date account.

The form does not constitute an appointment. For all appointments, please call the office at 845- 419-2122 and we will schedule you personally. We do not accept any walk-ins or emergency visits without prior consultations. If you are in need of emergency care, please call 911 or go to your nearest hospital.
